Appointments & Office Hours

The practice is open five days a week with Thursday evening and Saturday hours also available. Due to hospital surgical schedules and multiple offices, times may vary. We will do our best to accommodate your request.

Our Staff

If your insurance requires that you obtain a referral from your primary care physician in order to see a specialist, please contact them before making an appointment with our physicians.

If you have a work related injury, you must report it to your employer and obtain authorization. Then, either you or your employer may make the appointment.

PLEASE NOTE: You must have the physician referral documentation at the time of your visit. Co-payments are due at the time of the visit. THANK YOU.

Patients may also pay for our services at the time of the visit and submit bills to their insurance companies for reimbursement if we are not participants of their plan.

Deductibles and co-insurance vary greatly between plans and insurances may require pre-certification or pre-authorization for surgery. There are often higher co-pays for braces, devices and other Durable Medical Equipment (DME). We ask that you check with your insurance company or your employer regarding your co-pays and other patient responsibilities and be prepared to pay at the time of service. Our billing department is more than happy to help you with your billing questions.

For your convenience we accept Visa, MasterCard and American Express as payment options as well as personal checks, cash and money orders.
