Patients

We accept most major insurance plans.  If you are unsure if we accept your insurance, please call us.  It is your responsibility to verify with your insurance provider that you are covered.  Please keep in mind that many insurance plans require a referral from the insurance company and your primary physician.

Once you arrive at our office, you will be asked for a current copy of your insurance card, and you will be required to verify all relevant information.  

We will file your insurance claim for you, however, you are responsible for paying any deductible, co- insurance, or any other balance that is not paid by your insurance company.  

Copays are due at the time of your first appointment. Outstanding balances from your appointment are due upon receipt of your first statement.  We accept cash, checks, Mastercard, Visa, Discover and American Express.  We also participate in CareCredit.   (www.carecredit.com)
